Typically, a primary Health Care Provider falls into one of the following categories: Medical Doctor (MD) Doctor of Chiropractic (DC) Doctor of Osteopathic (DO) Doctor of Dentistry (DDS or DMD) Optometrist (OD) Naturopathic Physician (ND) Doctor of Veterinary Medicine (DVM) Special Provision: Retired primary Health Care Providers may be approved by submitting verification from their last licensing agency stating that they retired as a member in good standing. All other applicants must submit a current license or certification from the state or province in which they practice and a statement verifying that they are practicing within the regulations of their state, province of locality and personally administer care and/or counsel to an active patient or client base.
